Medical Declaration
All of the practical courses we run require a certain level of physical activity and the waves and wash the vessels pass over/through will create impacts on the body. If you have any concerns about the participant’s ability to take part in the course due to health or fitness reasons you should consult with their doctor and consider whether the course is appropriate.
Details of any medical treatment being received, or conditions that we may need to be aware of, if the attendee is taken ill during a course MUST be recorded in the sections for each attendee.
You agree that you have read and understood the above statement and declare that to the best of your knowledge they are fit to participate in the course.
Additionally: As the parent / guardian/carer of the course attendees names in the next section you give permission to the Instructors/staff of Powerboat Training UK/Aquasafe Powerboat School/Marine Education to administer any relevant treatment or medication to the named participant, when/if necessary. You agree to inform the school of any known conditions and medication requirements.
In addition, if the need arises, you authorise the members of staff to take the named individual to hospital and give full permission for any treatment required to be carried out in accordance with the hospital’s diagnosis. You understand that you will be notified, as soon as practical, of the hospital visit and any treatment given by the hospital.
